Interview leadership to evaluate readiness, commitment, support, resources and current employee environment. Program is outlined, industry benchmarks set and high-level goals established.

2) Assess

Establish baseline of current workforce health through tools such as a worksite inventory, employee needs assessment onsite biometric screening, health risk assessments, focus groups, employee interviews and current claims data.

3) Design & Implement

Roadmap detailing the integrated long-term, end-to-end plans and programs.

4) Measure & Report

Calibrate the roadmap as appropriate and deploy other wellness programs and campaigns as needed.

Health Risk Management

Our comprehensive Health Risk Management (HRM) program includes a web portal that provides employers with robust reporting and sophisticated behavioral change tools. Vivacity’s HRM program provides employees and dependents with a Personal Health Assessment and interactive, online tools that let them track and monitor their health.

Health Coaching

As part of the HRM program, Health Coaching supports and assists employees in identifying their individual needs and setting measurable, personalized goals.

Tobacco Cessation

Vivacity’s Tobacco Cessation program can help your employees stop smoking. We’ve partnered with the American Cancer Society (ACS) to give your employees and their dependents access to the ACS’s highly-successful Quitline®. These supportive, scheduled counseling calls by specially-trained tobacco cessation counselors help your employees stay on track. The program also provides Nicotine Replacement Therapy (NRT) for those employees who are medically qualified to receive it.

Worksite Biometric Screening

This service lets employees know their personal health indicators — information essential for behavior change and for understanding individual risk for serious chronic disease. Your employees’ personal health literacy is developed with the awareness of health values such as Body Mass Index (BMI), blood pressure, blood sugar and cholesterol. Vivacity has partnered with Summit Health Incorporated, a recognized industry diagnostic screening vendor, to provide onsite biometric screening events. We offer biometric testing packages which provide key health screening metrics for identifying health conditions that require immediate follow-up, as well as an annual measurement useful as a component of their personal health assessment.

Evaluation

Your wellness advisor will review the baseline data gathered from the Personal Health Assessment, employee interest survey and your benefit strategy to identify the programs that will work best for your organizations. Post-program assessments will be conducted to measure program effectiveness.

Reporting

Vivacity delivers aggregate reports on outcomes, utilization and employee satisfaction with the programs. Some standardized reports can be customized to suit your specific needs.
